$25 – single person
$40 – 2 people
$60 – up to a family of 4
$10 – per person additional cost after a family of 4 (Additional persons added to the membership must be in the same household)

- All additional x-rays discounted to 20%
- Opalescence Go bleach package included if paid in full for the year
- 20% off crowns, bridges, and dentures
- 15% off Root canals, implants, periodontal cleanings, filling
